Flat Feet in Children: Symptoms, Exercises & Corrective Therapy

Many parents notice their child’s feet turning inward when walking or that their shoes wear down unevenly along the inner edges. Flat feet (pes planus) are very common in toddlers because infants are born with a fat pad in the arch area. However, as children grow and start walking, a normal medial arch should gradually develop.

When Should Parents Seek Professional Evaluation?

By age 5 to 6, a flexible arch should begin to appear when standing on tiptoes. Evaluation by a physical therapist is recommended if your child exhibits:

  • Complaints of foot, shin, or knee pain after playing or walking;
  • Frequent fatigue or refusal to participate in sports;
  • Inward ankle collapse (calcaneal valgus);
  • Awkward or clumsy gait patterns.

Ignoring flat feet with poor foot mechanics can alter lower-limb alignment, putting unnatural stress on knees, hips, and the developing spine.

How Physical Therapy Corrects Flat Feet

Orthotic insoles support the arch passively, but they do not strengthen weak muscles. Active pediatric physical therapy is essential:

  • Intrinsic Foot Muscle Activation: Fun, game-based exercises that strengthen arch-supporting muscles.
  • Proprioceptive & Balance Training: Exercises on textured mats, balance beams, and unstable surfaces to stimulate neuromuscular arch control.
  • Calf & Achilles Stretching: Lengthening tight calf muscles that force the foot into pronation.
